It's the day that you made the decision to fully invest in yourself, your business and your future. 
It's the day you opened up, shared your dreams, identified where you currently are, and decided to get moving again. 
It's the day you begin to scale new heights in the Platinum Mastermind Program.

The Platinum Mastermind Program application begins with this questionnaire. 
You'll notice that this questionnaire is different from others you may have seen before - it's detailed, it's thoughtful, and it's built to help you think through your life both personally and professionally.
It's also built to help the Farm Owners Academy team and me understand your world and assist you in moving forward faster.

This questionnaire concentrates largely on your current reality, your business and what you hope to achieve from your time with the Platinum Mastermind Program.

So, to begin our relationship, we'll give you some sentence-completion activities throughout this questionnaire. 
When you complete these simple statements, you will help us understand your world a little better and enable all of us to move forward more quickly. 

Please be honest and open in completing this questionnaire. 
If the questions seem leading or assumptive at times, that's because they are; please don't take offence in any way if the statement doesn't exactly apply to you and simply answer from your gut (or, better yet, from your heart). 
Also, please understand that some of these questions may stir strong emotions, so give yourself time to complete them and begin from a place of strength and sureness that you are on the right path to breaking through.

Your application will be confidential and never shared outside of Farm Owners Academy. 
The entire goal is to give you and our team a little more clarity about where you are and where you want to go. 
You'll also give us the insight we need to help you leap into a land of greater possibility and abundance.
